I've always had this vague idea that spillover was limited to my home region and its neighbours - but if not, that'd be very useful...

Popularity will always spillover to other regions when the various triggers are reached, regardless of what causes them. If you create a blank promotion with no popularity, then on day one edit a random region to 100% and advance a day, all the spillover regions for the now-100% regions will have their spillover. Then if you advance another day, all the spillover regions for all the spillover regions will have their popularity adjusted too, and so on until there's no spillover left. So, in short; yes. ¬_¬
